ENT/Otolaryngology
Evan S. Greenbaum, MD.
ENT/OTOLARYNGOLOGY
Evan S. Greenbaum, MD, is a board-certified otolaryngologist with expertise in treating disorders of the ear, nose, and throat (ENT). Dr. Greenbaum’s philosophy of care is to do the right thing, at the right time, for the right reason. He treats each patient as he would a family member, with compassion and honesty.
Dr. Greenbaum’s special interests include endocrine surgery, salivary surgery, head and neck cancer, cutaneous cancer of the head and neck, breathing disorders, nasal obstruction, and voice changes. At Weiss Memorial Hospital, he provides outpatient surgery and clinic-based procedures focused on improving breathing and alleviating nasal obstruction.
Dr. Greenbaum earned his medical degree from Loyola University Stritch School of Medicine, Maywood, IL, and completed his residency at Loyola University Medical Center, Chicago, IL. He went on to complete a fellowship in head and neck surgical oncology and microvascular reconstruction at Case Western Reserve University/ University Hospitals Cleveland Medical Center. Additionally, Dr. Greenbaum serves as an assistant professor, Northwestern University Feinberg School of Medicine, Chicago, IL and oversees residents at John H. Stroger Hospital of Cook County.
He is a fellow of the American College of Surgeons and a member of the American Head and Neck Society and Chicago Laryngological and Otological Society. Published numerous times in scientific journals, Dr. Greenbaum’s research focuses on improving care and clinical outcomes.
Family Medicine
Rajdeep Dhami, M.D., M.P.H.
FAMILY MEDICINE PHYSICIAN
Rajdeep Dhami, MD, MPH, is a board-certified family medicine physician with expertise in adult medicine and routine pediatric care. Dr. Dhami believes a team approach, focused on educating and involving patients in their care plans, helps achieve optimal health results. She places special focus on preventive care that emphasizes an active lifestyle with exercise, healthy diet, regular primary care checkups, and routine screenings and vaccinations.
Dr. Dhami earned her medical degree from Saint James School of Medicine, Bonaire, NA, and completed her residency at Southern Illinois University, Decatur, IL, where she received training in both outpatient and inpatient settings, nursing homes, and in-home care. She went on to earn a Master of Public Health from Benedictine University, Lisle, IL, and completed an internship at the American Lung Association. Dr. Dhami believes in the importance of working at a community level to build education, promote healthy lifestyles, and combat preventable diseases and illnesses.
She is the recipient of the Ellis Island Medal of Honor and is a member of several professional organizations, including the Society of Hospitalist Medicine, Illinois Public Health Association, and the American Academy of Family Physicians.

Barry M. Summers, M.D., Ph.D., FACS
GENERAL SURGEON
Barry M. Summers, M.D., Ph.D., FACS, earned his dual MD/PhD degrees from The Chicago Medical School. He went on to conduct his general surgical residency at The University of Chicago (UC) Hospitals where he spent two years studying myocardial reperfusion injury. He then completed his last two years of general surgery residency at Mount Sinai Hospital and Medical Center. After residency, Dr. Summers served as an attending trauma surgeon at Mount Sinai Hospital. He then left to practice privately and has more than 20 years of surgical experience. Dr. Summers is board-certified by the American Board of Surgery and is a Fellow of the American College of Surgeons.
Gynecology
Nichole Butler, M.D.
GYNECOLOGIST
Nichole Butler-MooYoung, MD, is a seasoned gynecologist with a passion for education and communication. Dr. Butler has been a strong advocate for advancing the gynecological care of women during their reproductive years and beyond since the early days of her practice. She prides herself in advancing her clinical and surgical skills in parallel with the division of obstetrics and gynecology. Dr. Butler is board certified and a fellow of the American College of Obstetrics and Gynecology.
Dr. Butler completed her undergraduate education at Howard University and received her medical degree from the University of Illinois College of Medicine, Chicago campus. She completed her residency in obstetrics and gynecology at Rush University Medical Center, graduating with distinction in June of 2000 as a benign obstetrician gynecologist. Since graduation, Dr. Butler has provided care for women primarily as a private practitioner in a smaller group practice.
Dr. Butler’s special interests include fibroids, pelvic pain and sexual dysfunction. She is an aggressive minimally invasive surgeon with strong diagnostic skills. Dr. Butler excels in communicating with a diverse patient population and is focused on helping her patients become breast aware and reinforcing the importance of wellness examinations

Paul Abbo, M.D.
INTERNAL MEDICINE PHYSICIAN
Dr. Abbo earned his medical degree at the Medical University of Ohio. He completed his internal medicine internship and residency at the University of Illinois in Chicago and received special training in the treatment of difficult-to-heal wounds and cuts through the Advanced Wound Management and Hyberbaric Oxygen Therapy Program at Ohio State University.

Jeannette Kelly, D.O.
PHYSICIAN, CHICAGO CENTER FOR ORTHOPEDICS
Jeannette Kelly, D.O., is a non-surgical, osteopathic sports medicine physician. She cares for patients of all ages, from the pediatric to geriatric population, whether it be sprained ankles, fracture care, or arthritis management. Dr. Kelly takes a holistic approach to medicine, embracing osteopathic philosophies. She routinely practices Osteopathic Manipulative Medicine (OMM) on her patients, as another treatment option to help manage pain, and to prevent injuries. OMM can also be used to treat chronic headaches, pregnancy-related pain, carpal tunnel symptoms, as well as common colds.
Dr. Kelly is board certified in Family Medicine and in Sports Medicine. Along with treating patients in the clinic, she is a team physician for Windy City Thunderbolts and Schaumburg Boomers, South Suburban College, Robert Morris University, Saint Ignatius College Prep and Saint Rita of Cascia High School. She continues to participate in organized sports and endurance activities, and finds these experiences to be beneficial in serving her patients.
Dr. Kelly is currently accepting patients with musculoskeletal complaints, from sporting or work injuries to aches and pains. Patients are also welcome to see her for OMM treatments, weight loss management, exercise prescriptions and more.

Sebastian Ko, M.D.

Sebastian Ko, M.D., is a fellowship-trained orthopedic surgeon who specializes in arthroscopic and reconstructive techniques of the shoulder, knee, and hip, including shoulder replacements and hip arthroscopy. Dr. Ko’s practice philosophy emphasizes patient-centered care. He is dedicated to the successful diagnosis of each patient’s orthopedic concern and develops individualized treatment plans that include both non-surgical and surgical options.
Dr. Ko is a former NCAA Division 1 athlete with a strong interest in the treatment of athletic injuries. His experience includes treating elite athletes while serving as theassistant team physician for the Chicago Blackhawks, Chicago Cubs, and Rockford IceHogs professional sports teams, and the Northwestern Division 1 collegiate football team.
Dr. Ko’s practice philosophy emphasizes patient-centered care. He is dedicated to the successful diagnosis of each patient’s orthopedic concern and develops individualized treatment plans that include both non-surgical and surgical options.
Born and raised in Vancouver, Canada, Dr. Ko completed medical school and his residency training in Orthopedic Surgery at the University of British Columbia. After residency, he completed a clinical fellowship in Orthopedic Sports Medicine and Arthroscopy at Northwestern Memorial Hospital/Northwestern University. He is dual board-certified by the American Board of Orthopaedic Surgery and the Royal College of Physicians and Surgeons of Canada (FRCSC).
